How do I read a QR code from an image?

What types of data can a QR code contain?

Why can't the reader decode my QR code?
Common causes: image is too blurry, the QR code is at an extreme angle, there's insufficient contrast, or the quiet zone (white border) is cropped. Try a clearer screenshot.
